If you want to save time, you can request a follow up call filling the following form from home.
Public Health Canada is enforcing a period of 14 days of self-isolation at home after your arrive. Call to re-schedule your appointment to meet the restrictions.
You must be free of infection symptoms for your visit (cough, fever, malaise, etc).
