Cancellations
We require 48 hours for all cancellation of appointments. If you are more than 15 minutes late, we may ask you to reschedule your appointment.

Appointment Requests
I am a new patient and would like to be seen.
All patients that would like to be seen in our clinic must have a referral signed by your primary care physician.
Once we have it, we can book your appointment. Our current wait time for an appointment is approximately 4 weeks.
Please Bring your Ontario Health Insurance card and photo ID when you come in for the consultation or treatment.
I am an existing patient and need another appointment.
Please fill out the Online Booking Request Form and one of our Patient Care Coordinators will contact you to book your appointment.
