We accept all referral forms for adults 18+ accompanied by a physician's licence and billing number to obtain OHIP coverage.

Primary Care Provider Role

Family Physicians or Primary Care Provider play an active role in the treatment of chronic pain patients, and the referred patient must have one. 

We will provide an assessment and a treatment plan for your patient’s chronic pain problem. In some cases, the treatment may be initiated by our clinic, however, once stabilized the patient will be returned to you for ongoing care. This may include ongoing pharmacotherapy recommendations, diagnostic testing, or further referrals.

Most procedures do not require stopping anticoagulants.  

However, if your patient is on anticoagulation, please include that information with the consult request.

Epidural injections require anticoagulation temporary hold.

  • Use our referral or your EMR's.  
  • Send pertinent images of painful area.
  • If the patient is on anticoagulants, advise if stopping is applicable. 
  • Make sure to have your CPSO and billing number on the referral for acceptance.

Treatment Options

  • We offer interventional pain care consisting of minimally invasive treatment with anesthetics, analgesics, and neurolisis for chronic pain relief.
  • "Regenerative" treatments with Platelet Rich Plasma injections for ligament, tendon or joint injuries are available.
  • Our clinic does not endorse or oversee opioid prescriptions, 
  • The clinic does not oversee treatment of patients with abuse or overuse of opioid related pathologies.
  • We specialize in non-surgical treatment of chronic pain.
